For multi-site disconnect requests please include a spreadsheet referencing the above information.
If you have not provided this information, please respond back to this email. Once the information is received PureIP will confirm that the request is complete and that we do not require any additional information to process your request. We will notify you once we determine that the form is complete, and the request is accepted. Your service(s) will be disconnected within 30 days from date of acceptance. Once the disconnect acceptance date is set, you will receive an email notification from one of our Order Management Systems (OMS) with the Disconnect Order number.
Please refer to your agreement with PureIP including the Standard Terms and Conditions for information related to your contract and any continuing obligations (including Early Termination Fees) arising from your disconnect of your service(s).
